As an RN Case Manager at Access To Home Care Services, Inc.
, you will play a pivotal role in promoting patient-centered care, improving patient outcomes, and enhancing the overall quality of life for patients receiving healthcare services in their homes.
Responsibilities:
Conduct thorough assessments of patients' health conditions, medical histories, and home environments to develop individualized care plans.
Collaborate with physicians, therapists, social workers, and other healthcare professionals to design and implement effective care strategies.
Monitor patients' progress regularly, update care plans as needed, and ensure appropriate interventions are in place.
Perform admissions, recertification, and resumptions of care.
Maintain accurate and timely documentation of all patient interactions and provided care in accordance with Department of Health standards and regulations.
Coordinate medical equipment, supplies, and other resources essential for patients' well-being and comfort at home.
Actively participate in case conferences, team meetings, and performance improvement initiatives to enhance the quality of care provided.
Requirements:
Valid and unrestricted Registered Nurse (RN) license.
Strong clinical assessment skills and the ability to develop effective care plans.
Excellent communication, interpersonal, and leadership skills.
Empathetic, compassionate, and patient-centered approach to care.
Ability to work independently and collaboratively as part of a multidisciplinary team.
